Oxford Partners with Bridges Palliative Care for Chronic Illness Support

Oxford Rehabilitation & Healthcare Center has partnered with Bridges Palliative Care to introduce a new Chronic Illness Support model for its skilled nursing facility. The model, based on a platform by Marquis Health Consulting Services, emphasizes a patient-centric approach, integrating palliative care and pastoral support with specialized clinical programming. The program focuses on physical and spiritual wellness for chronically ill patients, providing emotional and spiritual support, disease-specific education, health coaching, social services support, life enrichment activities, and community connections. The goal is to improve mental health, well-being, health outcomes, and reduce hospitalizations for this patient population. Oxford Enhanced Senior Living campus offers various skilled nursing and personal care units, further enhancing their commitment to progressive care.
